Application for Free or Reduced Lunch
Parents,
In order to qualify for free and reduced lunch, please print, complete, and return this form. You can either email it gail.miller@nhcs.net or drop it off at Laney. It must be returned by Sept 25. This will be needed to receive free meals using our drive by/drop off program.
Note: A new application must be filled out each year. If you received free or reduced lunch last year, you must re-apply each year.
